The Grand County Sheriff’s Office identified Lewis as one of the victims of Sunday’s accident, Utahin a statement. The Sheriff’s Office confirmed the incident occurred in the Mine Bottom area of Utah’s Grand Canyon. Authorities have not released other details about how the crash occurred.

Andy Lewis performed a stunt on August 28, 2022, completing 143 sideways surf moves in 60 seconds to put his name in the Guinness Book of World Records. Lewis performed stunts at Diaoshuilou Falls in Jingbo Lake, China.

Grand County Sheriff Jamison Wiggins confirmed to The Moab Sun that Lewis ran a BASE jumping and local guiding operation in Moab. It’s called BASE Jump Moab and is owned and operated by the 39-year-old.
