Who is Ibrahim Kaldoun Hilmi?

Ibrahim Khaldoon Hilmi is a fugitive accused of helping orchestrate a $3.7 billion Medicare fraud scheme, one of the largest in U.S. history. He is now in U.S. custody after authorities found him in Türkiye and flew him back to face charges, the FBI announced Monday.

Hilmi fled the United States in May 2025 and was recently detained by Turkish authorities, according to the FBI. The FBI’s Critical Incident Response Team then flew to Türkiye and returned Hilmi to the United States on Friday through a foreign custody transfer operation, Fox News reported.

Kash Patel calls Hilmi’s return a ‘huge win’ in the fight against fraud

“Ibrahim Khaldoon Hilmi was charged with one of the largest Medicare fraud cases in history, allegedly orchestrating a massive $3.7 billion scheme to defraud Medicare. He has been on the run since May 2025, but we caught him,” Patel said in a statement.

Patel also posted about the arrest on X and praised the Miami FBI, Department of Justice and Turkish authorities for their roles in the operation. “Thanks to the outstanding work of the FBI in Miami, the Department of Justice, and our partners in Turkey, Hilmi was arrested overseas and, after being transferred to custody abroad, returned to the United States to face justice,” he wrote.

He added that the arrests were part of a larger effort to combat fraud. “This is another huge victory for the FBI and the White House task force led by Vice President Vance in their fight against fraudsters, and a huge win for the Trump administration, demonstrating that any criminal who steals from American taxpayers will be caught, no matter where they try to hide,” Patel wrote.

He also thanked U.S. Ambassador to Turkey Tom Barak, saying: “A special thank you to Ambassador Tom Barak, who continues to be a valuable partner – without his tireless efforts, this case would not have been possible.”

Hilmi’s arrest marks the second high-profile return in less than a week for a fugitive suspected of Medicare fraud.

As of Thursday, the FBI announced it had captured Herbert Kimble, who had been on the run since 2024 and allegedly masterminded a separate Medicare fraud scheme worth an estimated $1.3 billion. Together, the two cases involve approximately $5 billion in fraud against taxpayer-funded health care programs.
