New Delhi: In a bid to enhance last-mile healthcare and improve response during medical emergencies, the Center on Monday launched a mobile app for community health officers (CHOs) to enable real-time patient management and speed up referrals at Ayushman Bharat Health and Wellness Centres.The app was launched by the Federal Ministry of Health to digitize clinical workflow at the primary care level. It will allow CHOs to register patients, access medical history and update treatment records in real time, replacing manual processes that often slow down care delivery.The platform will also help identify high-risk cases and emergencies early, allowing frontline providers to make faster referrals to higher facilities, officials said. This is expected to be critical in conditions such as cardiac events, stroke, maternal complications and serious infections, where timely intervention can significantly improve outcomes.Public health experts note that delays in documentation and communication at the primary care level can impact routine care and emergency response. With instant access to patient data and standardized digital records, the app helps CHOs track patients with chronic conditions more effectively, ensure continuity of care and flag complications early.The launch is part of the government’s wider digital health push under the Ayushman Bharat Digital Mission, which aims to build a comprehensive health data ecosystem across the country. By digitally connecting primary care providers with higher centers, the program is expected to streamline referrals and improve coordination between different levels of care.The app will also enhance oversight, allowing administrators to monitor service delivery patterns, identify gaps and assess response timelines, including during emergencies, officials added.India currently has over 160,000 Ayushman Bharat health and wellness centers serving as the first point of contact for a large portion of the population, especially in rural and underserved areas. CHOs are typically trained nurses or practitioners who play a key role in providing preventive, promotional and basic therapeutic services in these settings.
